Immunizations
All applicants to Middle Georgia State University per USG Board of Regents policy are required to submit proof of immunization prior to attending classes on campus. Applicants applying for online programs may select the distance learning exemption by completing the Certificate of Immunization and checking the appropriate exemption category at the bottom of the page. Other limited exemptions may apply as outlined on the form.
Check your specific program requirements for appropriate immunization guidance, as some programs have immunization requirements beyond the minimum for in-person attendance.
All students attending in-person classes must submit acceptable proof of immunization against the following: 2 MMRs (Measles, Mumps, and Rubella), which is not required if born before 1957; 1 Tdap (Tetanus, Diphtheria, and Pertussis) vaccine within the past 10 years; 2 doses of Varicella (chicken pox) vaccine or history of disease, which is required for all US born students born in 1980 or later, and which is required for ALL foreign-born students, regardless of year born; and 3 doses of Hepatitis B vaccine for persons aged 18 or younger at time of expected matriculation.
